Duchossois Center for Advanced Medicine (DCAM) located at 5758 S. Maryland Ave, Chicago, IL.

Parking
A convenient valet parking service is available at the circle drive on 58th street in front of the DCAM outpatient facility for a nominal fee to patients and visitors from 6:30AM to 6:00 PM Monday through Friday.

Wheelchair accessibility.
A wheelchair is available outside the Main entrance to DCAM. A wheelchair ramp is also located here. Please feel free to bring your own wheelchair.
 
Clinic Cost and Referral
The cost of the initial visit is approximately $450.00. This fee covers
the neurological history and examination. There are additional charges
for tests that may be required as well as for any referrals. These fees
are usually covered by insurance.

Please validate insurance coverage and obtain referral forms if required
by your insurance carrier prior to your visit.  Please verify that Dr.
Gomez is covered by your insurance or you will receive a bill for
service after your visit. University of Chicago does accept Medicare
assignment.
